An Intake Form and an Informed Consent and Disclosure Form must be completed prior to the first massage. It is suggested that individuals come to their first massage appointment 10-15 minutes early in order to fill this out prior to the massage. Otherwise, the massage time will be shortened. You may also fill it out and submit it online prior to your appointment or print a hard copy and bring it with you to your appointment.
